Established in 2008, Piedmont Health SeniorCare provides an alternative to nursing home care to residence of Alamance, Caswell, Chatham, Lee and Orange counties; it allows nursing-home eligible seniors to remain in their homes and communities.

Nationally, programs of all-inclusive care for the elderly (PACE) sites are centered on the belief that elderly individuals with chronic health care needs and their families are best served in their homes and communities whenever possible.

Piedmont Health SeniorCare is a PACE program that offers an innovative, financially viable option for community-based, comprehensive long-term care for the nursing home eligible population. It builds on existing community services to offer a full-continuum of care by an interdisciplinary care team in collaboration with patients and their loved ones.

According to the National PACE Association, the typical participant has the following characteristics:

–> The average age is 80 years.
–> 75% of participants are female
–> The average participant has 7.9 medical conditions (many chronic)
–> 2.7 limitations in Activities of Daily Living such as grooming, feeding and bathing
–> 49% of participants have been diagnosed with some form of dementia
–> 93% of participants remain at home
