UNC Health Care

UNC Health Care is a major referral center for Piedmont Health.  We partner closely on the Carolina Health Net program to better manage the needs of the low-income, uninsured population of our shared service area.   The program provides a medical home at Piedmont Health and offers an innovative care management model, with specialty care provided by UNC.  Piedmont Health also provides facility space for operations of the UNC Student Health Action Committee (SHAC) Free Clinic.  

Alamance Regional Medical Center

Piedmont Health partners with ARMC for specialty care for its patients and several projects to improve care of the medically under served populations, including development of Piedmont Health’s Piedmont Health SeniorCare Program with funding from The Duke Endowment, and operations of ARMC’s Open Door Free Clinic of Alamance County.

Foundations

Blue Cross and Blue Shield of North Carolina Foundation

Support from the BCBSNC Foundation has been instrumental in enabling several Piedmont Health initiatives, including the development of the Piedmont Health SeniorCare program, a strategic brand marketing project, a safety-net dental care technical assistance program, and a collaboration with UNC to embed a cardiologist in a primary care practice. The Kate B Reynolds Charitable Trust

Piedmont Health is current grantee of the Trust, which supports its Diabetes Management program.

Susan G. Komen for the Cure NC Triangle to the Coast

Project Access provides hundreds of Piedmont Health patients with access to free mammography screening and follow-up breast imaging at UNC Hospitals or on the Rex Mammography Mobile Unit.

Carol Woods Retirement Community

Piedmont Health is collaborating on Carol Woods’ Community Connections for Seniors program, funded by The Duke Endowment, to better coordinate care for the elderly population of Orange and Chatham Counties.

State & Local Government

North Carolina Office of Rural Health and Community Care

Piedmont Health is a member of Central Carolina Community Care Medicaid network, and additionally operates a migrant/seasonal farm worker outreach program funded by theNCORHCC’s Farmworker Health Program.

North Carolina Women’s Health/Reproductive Services Branch

With Title X funds provided through the Latino Family Planning Demonstration and Long Term Contraception grants, Piedmont Health provides approximately 140 under/uninsured women with free access to long-term contraception methods each year.

Orange County Department of Health

Piedmont Health partners with health department leaders to offer a primary care home to refugees from Myanmar (formerly known as Burma).

Orange County Human Services Outside Agency Grant

Piedmont Health is fortunate to receive funds for its primary care services for the under served through the Human Services Agency funding stream.

Orange County Partnership for Children

Orange County Partnership for Children supports a bilingual (English/Spanish) Child Resource Coordinator at our Carrboro Health Center location.
