To receive services at a PHS Comunity Healt Center, you must be a registered
patient.
Payment for services is based on a sliding-fee scale according to family income.
PHS accepts many health insurance plans, including Medicaid and Medicare.
Please bring the following information with you to register at our clinics:
- 1) PROOF OF INCOME, for every household member. This may include:
- a paycheck or social security check, or
- a copy of your W-2 tax form reporting your income, or
- a bank statement if you receive direct deposit.
- 2) PROOF OF RESIDENCE, for example, a bill addressed to yourself sent
by mail.
- 3) EVIDENCE OF HEALTH OR DENTAL INSURANCE COVERAGE, if any
- 4) A SOCIAL SECURITY CARD, if you have one.
