Sliding Fee Scale
 
To receive services at a PHS Community Health Center, you must be a registered patient. Payment for services is based on a sliding-fee scale according to family income. PHS accepts most health insurance plans, including Medicaid and Medicare. To qualify for a sliding fee discount, please bring the following information with you to register at our clinics:
 
1. Proof of Income, for every household member. This may include:
  • a paycheck stub or
  • a copy of your tax form reporting your income, or
  • a bank statement if you recieve direct deposit.
2. Proof of Residence, for example, a bill addressed to yourself sent by mail.
3. Evidence of health or dental insurance coverage, if any.
4. A Picture I.D. card.
