During Your First Visit
We want to optimize your first time that you visit with the care team. The Care team will want to understand your complete medical history. If you are transferring from other provider or recently seen another medical provider, including a specialist, or have been seen in the emergency room or hospital, please bring any test results, care plan or discharge summary that you received. This is important information we want to add to your medical history. If you want us contact the provider to transfer your records to our practice please ask the operator to connect you to medical record department when you call. It is also important to bring all of your medications (prescriptions and over the counter medicines) to this first appointment. Optimal health will include our Care Team coordinating your care.
