Piedmont Health has a dedicated staff of more than 70 board-certified physicians, family nurse practitioners, physician assistants, dentists, hygienists and social workers. Our goal is to provide our patients with the highest quality care, dedication, satisfaction and safety.

We are a Joint Commission accredited organization. The Joint Commission is the primary accreditor of healthcare organizations in the United States. Today, Joint Commission accreditation of an ambulatory care organization is a widely recognized standard for evaluating and demonstrating high quality services. Many payers, regulatory agencies and managed care contractors require Joint Commission accreditation for reimbursement, certification and licensure, and as a key element of their participation agreements. Joint Commission accreditation represents the “gold seal” of approval in health care and provides the most comprehensive evaluation process in the industry.

We believe in using the best available scientific evidence in our decision-making, tools and practices. We benchmark ourselves against similar organizations in our outcomes for chronic diseases, patient satisfaction and office efficiency. We have electronic health records that allow us to collect and analyze patient data so we can provide better individualized care.

Your Rights

The Health Insurance Portability and Accountability Act (HIPPA) requires that healthcare providers follow certain rules to protect the privacy of patients’ health information. We respect your rights, values and dignity and ask that you recognize the responsibilities that come with being a patient. That’s our commitment to you. 

Notice of Information Privacy Practices

WE HAVE A LEGAL DUTY TO PROTECT HEALTH INFORMATION ABOUT YOU

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Piedmont Health Services may use or disclose Protected Health Information (PHI) about you in the following circumstances:

  • To provide health care treatment to you
  • To obtain payment for services rendered
  • For operation of our health care facilities
  • When the state or federal law requires it
  • To contact you with appointment reminders
  • To contact you with information about treatment or services

Where your Protected Health Information is concerned, you have the right

  • To object to certain uses or disclosures
  • To request restrictions on the uses or disclosure of information about you
  • To request different ways of communicating with you
  • To request copies of your health care information
  • To request an amendment of PHI about you
  • To see a list of disclosures we have made of your PHI
  • To request a copy of this notice

You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may not agree to it if it would adversely affect your care.

If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will comply with your request unless a law requires us to share that information.

Your health care provider has chosen to be a part of NC Health Information Exchange because of the many benefits of sharing health information electronically. You may choose not to participate (“opt-out”). Your choice to opt-out will not affect your ability to access medical care. You can opt out by calling, toll-free, 855-926-1042, online at www.nchie.org or by filling out the opt-out form available in your health care provider’s office.
