Piedmont Health Services, Inc. has been awarded a three-year grant by the Kate B. Reynolds Charitable Trust of Winston-Salem,

The company, which operates six community health facilities-including Prospect Hill Health Center, will receive $123,596/year.

“The funds will be used to develop and provide diabetes patient self-management education to low-income individuals and their families at the Prospect Hill Community Health Center (CHC) in Caswell County.”, said Heather Miranda, nutrition director, adding that the grant will be paid for three years.

“Our organization has been highly focused on improving chronic disease outcomes during the last 10 years,” Miranda said.

The grant from Kate B. Reynolds Charitable Trust will give us additional resources to develop an interdisciplinary model for doing group diabetes care. This model can be spread to all six of our community health center sites. This will serve approximately 3, 600 people with diabetes.”

Piedmont Health Services was founded in 1970 and serves over 40,000 patients. Prospect CHC is one of six community health centers operated by Piedmont Health Services. In addition to Prospect Hill in Caswell County, Scott Community Health Center, Drew Community Health Center and Piedmont SeniorCare are in Alamance County. Piedmont Health also operates Moncure Community Health Center and Siler City Community Health Center in Chatham County.

The Kate B. Reynolds Charitable Trust was created in 1947 by the will of Mrs. William N. Reynolds of Winston-Salem. Three fourths of the Trust’s grants are designated for use for health-related programs and services across North Carolina and one-fourth for the poor and needy of Winston-Salem and Forsyth County.
